左旋卡尼汀对维持性血透者疗效的观察

摘要

Objective The purpose of the observation was to study the curative effect of oral L-carnitine supplement in maintenance hemodialysis patients. Methods The changes in laboratory parameters and clinical manifestations were observed in 18 patients before and 3 months after L-carnitine was administered orally at a dose of 1 g· d-1. Results The serum level of camitine in hemodialyzed patients were significantly lower than those of the control group ( P < 0.001).After 3 months of Lcarnitine treatment, with the increased concentration of serum eamitine, physical startus, appetite, cardiac function, intro-dialytic hypotention and cramps were improved. And hematecrit, serum pro-albumin, transferrin and high density lipoprotein were increased, at the mean time total cholesterol was decreased. Conclusion Oral supplement of L-camitine in hemodialyzed patients appears to be associated with an improvement of nutritional status,cardiovascular function and elevation of dialysis tolerance .%目的：观察口服左旋卡尼汀对维持性血透病人的疗效。方法：给病人口服左旋卡尼汀，1 g·d-1，共3个月，观察治疗前后病人各项实验室检测指标的变化及临床表现。结果：血浆卡尼汀浓度治疗前显著低于正常人(P<0.001)，治疗1个月后有明显上升，3个月后升高更为显著。治疗后病人体力增强，食欲改善，血透中并发症减少，红细胞压积升高，血浆前白蛋白、转铁蛋白上升，总胆固醇下降，高密度脂蛋白升高。结论：应用左旋卡尼汀可有效地改善血透病人的营养状况及心血管功能，提高病人对血透的耐受力。
