A case of strongyloidiasis presenting as a colonic mass. A 74-year-old man from El Salvador who had a history of aortic valve replacement and was taking warfarin presented with 9 days of right-lower-quadrant pain, constipation, and weight loss. A computed-tomography scan of the abdomen revealed a mass in the ileocecal area. Colonoscopy confirmed an irregular 2-cm mass at the ileocecal valve. Given the patient's country of origin, a laboratory workup was done to check for parasitic infections. It revealed the presence of Strongyloides stercoralis antibodies. The patient was treated with two 200-ug/kg doses of ivermectin, administered on two consecutive days. Colonoscopy two weeks later showed complete resolution of the colonic mass.
