    • 摘要: [目的]探讨益气消毒方对炎症性贫血(anemia of inflammation,AI)模型大鼠造血功能,以及白细胞介素-6(interleukin-6,IL-6)、铁调素(hepcidin)水平的影响。[方法]40只Wistar大鼠中随机选取5只作为正常组,35只采用尾部皮下注射胶原乳剂的方法制备胶原诱导型关节炎(collagen-induced arthritis,CIA)大鼠模型,并在此基础上,连续7 d腹腔注射0.1μg·mL-1的IL-6溶液0.4 mL,诱导胶原诱导型关节炎伴炎症性贫血(collagen-induced arthritis with anemia of inflammation,CIA-AI)。将造模成功的20只大鼠随机分成模型组,益气消毒方低、中、高剂量组,各组每天灌胃给予相应药物,连续2周。检测大鼠外周白细胞(white blood cell,WBC)、红细胞(red blood cell,RBC)、血红蛋白(hemoglobin,HGB)、平均红细胞体积(mean red blood cell volume,MCV)和血小板(platelet,PLT)水平;计算比较肝脏和单侧股骨指数;以酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测大鼠血清及肝组织匀浆IL-6、hepcidin、铁蛋白和铁水平;苏木素-伊红(hematoxylin-eosin,HE)染色观察骨髓病理形态学变化。[结果]与正常组比较,模型组WBC、RBC和HGB水平明显降低,肝脏指数升高(P<0.05),单侧股骨指数显著下降(P<0.01);血清及肝组织匀浆IL-6、hepcidin、铁蛋白水平显著升高(P<0.01),铁水平明显下降(P<0.05);股骨骨髓中粒细胞和巨核细胞减少,髓腔内充填水肿的血管纤维脂肪组织。与模型组比较,益气消毒方高剂量组大鼠外周血WBC、RBC和HGB水平升高,单侧股骨指数升高,肝脏指数下降,血清及肝组织匀浆中铁水平显著升高(P<0.01),IL-6、hepcidin、铁蛋白水平下降(P<0.05);股骨骨髓病理形态改善。[结论]益气消毒方可能通过调控机体炎症及骨髓造血关键因子IL-6、hepcidin水平,发挥纠正CIA-AI模型大鼠贫血的功效。

    • 摘要: Haemochromatosis is a genetic disease caused by hepcidin deficiency,responsible for an increase in intestinal iron absorption.Haemochromatosis is associated with homozygosity for the HFE p.Cys282Tyr mutation.However,rare cases of haemochromatosis(non-HFE haemochromatosis)can also be caused by path-ogenic variants in other genes(such as HJV,HAMP,TFR2 and SLC40A1).A working group of the International Society for the Study of Iron in Biology and Medicine(BIOIRON Society)has concluded that the classification based in different molecular subtypes is difficult to be adopted in clinical practice and has proposed a new classification approaching clinical questions and molecular complexity.The aim of the present review is to provide an update on classification,pathophysiology and therapeutic recommendations.

    • 摘要: 目的:研究铁代谢指标对缺铁性贫血与Hepcidin之间的相关性.方法:选择2016年5月-2018年1月我院收治的84例血液病患者为研究对象,同时选择45例体检者为参照对象,在研究对象分析中,以患者是否发生贫血分为贫血组与非贫血组,其中,贫血组32例,非贫血组52例.实施血液检验分析,评估检验结果,观察网织红细胞及红细胞参数变化情况.结果:经研究证实三组研究人员的网织红细胞与红细胞参数对比有差异(P<0.05),贫血组低于非贫血组和对照组,数据差值比较结果显著.结论:研究证实,缺铁性贫血患者诊断评价过程中,以铁代谢指标作为评估标准,能够了解患者患病情况,对患者自身疾病筛查有一定帮助,可将该法推广到临床.

    • 摘要: 目的:分析贫血相关指标、铁代谢指标及炎症指标与H epcidin之间的相关性.方法:针对医院中所收治的血液病患者,来实施随机数字表的方法进行相关的筛选工作,并将其中部分患者采取随机纳入的方式到本次的临床试验之中,时间则选择在2016年5月-2018年1月之间,患者的总数为84例.根据患者有无贫血的症状将其分成发生贫血组和非贫血组,同时纳入45例健康的体检患者作为对照组,分析贫血相关指标、铁代谢指标及炎症指标与H epcidin之间的相关性.结果:根据此次的试验结果表明,贫血组和非贫血组中患者的Hb、MCH、MCHC等水平要低于对照组,而非贫血组的Hb、MCH、MCHC等水平要高于贫血组,也具备统计学意义,P<0.05.结论:在进行贫血的鉴别的过程中,贫血相关指标、铁代谢指标及炎症指标都有非常重要的应用意义,能更加精确的诊断贫血的类型,值得进行推广.

    • 摘要: 目的 探讨当归多糖(ASP)、香菇多糖(LEP)和蒲公英多糖(DP)3种中药多糖体外抑制肝癌HepG2细胞增殖的作用及其相关机制.方法 体外培养HepG2细胞,CCK-8和平板克隆实验检测3种多糖对HepG2肝癌细胞增殖能力的影响;GEO数据库分析Hepcidin在人肝癌组织中表达情况;qPCR和Western印迹检测三种中药多糖对Hepcidin表达的抑制作用.结果 与Con组相比,100、200、400 mg/L的ASP,LEP和DP均抑制HepG2细胞增殖(P<0.01).GEO数据库分析证实Hepcidin在人肝癌组织中高表达;200 mg/L的ASP和DP,400 mg/L的LEP诱导HepG2细胞48 h可显著下调Hepcidin的表达(P<0.05).结论 ASP、LEP和DP可能通过下调Hepcidin,降低铁负荷,进而发挥抑制肝癌细胞增殖的作用.

    • 摘要: 慢性炎症与肿瘤的发生发展有着密切关系,一些促炎因子构成的炎症微环境导致高表达铁调素,后者造成机体对铁的失利用并引起贫血.低氧诱导因子脯氨酰羟化酶(hypoxia-inducible factor-prolyl hydroxylase,HIF-PH)在胃肠道肿瘤中高表达,与肿瘤的侵袭、不良预后有关.因此针对HIF-PH的抑制剂(HIF-PHI)是一种有望替代红细胞生成机制的更安全、更有效和可获得的新型红细胞生成刺激制剂.

    • 摘要: BACKGROUND Hyporesponsiveness to erythropoiesis-stimulating agents(ESAs)is a prevalent problem in patients with chronic kidney disease.It is associated with increased morbidity and mortality in patients who undergo dialysis.A significant proportion of patients do not respond to iron supplementation and conventional ESAs.We report a case of severe ESA hyporesponsiveness-related anemia that was successfully treated with oral roxadustat.CASE SUMMARY A 59-year-old Chinese woman had high blood glucose for 25 years,maintenance hemodialysis for 7 years,and recurrent dizziness and fatigue for more than 2 years.Laboratory tests showed severe anemia(hemoglobin level of 54 g/L),though bone marrow biopsy,fluorescence in situ hybridization,and hemolysis tests were within normal ranges.We initially administered first-line therapies and other adjuvant treatments,such as blood transfusions,ESAs,and adequate dialysis,but the patient did not respond as anticipated.Her erythropoietinresistant anemia was probably not only due to chronic renal insufficiency.The patient received the hypoxia-inducible factor prolyl hydroxylase inhibitor roxadustat(100 mg,three times weekly).After 12 wk of treatment,the patient’s hemoglobin increased significantly,and her symptoms were alleviated.During the follow-up period,adverse drug reactions were controllable and tolerable.CONCLUSION Oral roxadustat is effective and tolerable for the treatment of ESA hyporesponsiveness-related anemia in patients undergoing hemodialysis.

    • 摘要: 目的:分析贫血H epcidin指标和缺铁性贫血和铁代谢指标的相关性.方法:挑选本院2016.05——2018.01时间段内收治的患有患有血液病患者共计84名,按照实验对象临床反应显示将其分成贫血组、非贫血组,适应临床统计报告分别命名为常规组(32名)与实验组(52名),同期选择健康体检者45名登记为健康组.随之为印证缺铁性贫血、铁代谢指标和H epcidin之间的相关性,此时展开临床技术研究分析.结果:健康组对象的H epcidin浓度指数相较于常规组与实验组之间存在一定差异性,且相较结果比较可见皆比健康组更差,P<0.05.相比健康组,常规组与实验组各项铁代谢指标相差明显,P<0.05.结论:在进行贫血的鉴别的过程中,贫血相关指标、铁代谢指标及炎症指标都有非常重要的应用意义,能更加精确的诊断贫血的类型,值得进行推广.

    • 摘要: Bleeding and altered iron distribution occur in multiple gastrointestinal diseases,but the importance and regulation of these changes remain unclear.We found that hepcidin,the master regulator of systemic iron homeostasis,is required for tissue repair in the mouse intestine after experimental damage.This effect was independent of hepatocyte-derived hepcidin or systemic iron levels.Rather,we identified conventional dendritic cells(cDCs)as a source of hepcidin that is induced by microbial stimulation in mice,prominent in the inflamed intestine of humans,and essential for tissue repair.cDC-derived hepcidin acted on ferroportin-expressing phagocytes to promote local iron sequestration,which regulated the microbiota and consequently facilitated intestinal repair.Collectively,these results identify a pathway whereby cDC-derived hepcidin promotes mucosal healing in the intestine through means of nutritional immunity.
